VISITS
After speaking to a Hospice Brazos Valley admissions professional, a nurse will be scheduled to meet with you wherever you call home. That team member will sit with you and your family to listen to your needs, answer questions, and address concerns. The goal of this first visit is to get to know you, learn about your unique needs, and discuss how hospice care can help you achieve your goals. This visit may lead to admission, or it can be the first step in learning more about what options are available to you and your family.
Also, during the initial visit, you will be provided with an admission booklet that will provide your family with answers to common questions and an introduction to your hospice team.
PLAN OF CARE
Once a Hospice Brazos Valley admissions nurse meets with you and completes your initial assessment, a plan of care is developed based on your goals and is coordinated with your physician and our care team. If you do not have a primary physician, our medical director can oversee your care. Services can begin the same day you contact our office.
